Dihydropyridines are calcium channel blockers e. g. nifedipine, nitrendipine, amlodipine – mostly used in the treatment of hypertension. They also have coronary vasodilatory action and hence useful in the treatment of angina pectoris. But since they produce reflex tachycardia, only long acting preparations should be used, preferably in combination with beta blockers, for treatment of angina. Otherwise reflex tachycardia can be deleterious to those with angina as it worsens myocardial ischemia due to increased demand for oxygen. In those with pre-existing bradycardia, dihydropyridines can be used without adding beta blockers.
